Hlavn\u00ed h\u00e1k m\u00e1 jmenovitou nosnost 20 tun a pomocn\u00fd h\u00e1k m\u00e1 jmenovitou nosnost 5 tun s v\u00fd\u0161kou zdvihu 9 metr\u016f. Je\u0159\u00e1b je ovl\u00e1d\u00e1n pomoc\u00ed d\u00e1lkov\u00e9ho ovl\u00e1d\u00e1n\u00ed ze zem\u011b. V\u00fdrobu i instalaci provedla jist\u00e1 spole\u010dnost zab\u00fdvaj\u00edc\u00ed se je\u0159\u00e1bnickou technikou. Instalace za\u010dala koncem \u0159\u00edjna 2022 a zved\u00e1n\u00ed bylo dokon\u010deno za\u010d\u00e1tkem listopadu. V dob\u011b nehody je\u0159\u00e1b nepro\u0161el kontrolou a certifikac\u00ed a nebyl registrov\u00e1n k pou\u017eit\u00ed.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"accident-description\">Popis nehody&nbsp;<\/h2>\n\n\n\n<p>Jednoho dne v prosinci 2022, kolem 19:00, obsluhoval sl\u00e9v\u00e1rensk\u00fd d\u011bln\u00edk Lin elektrick\u00fd je\u0159\u00e1b ve sl\u00e9v\u00e1rn\u011b a pokou\u0161el se sou\u010dasn\u011b zvednout t\u0159i p\u00edskovi\u0161t\u011b. St\u00e1l mezi zvedan\u00fdmi p\u00edskovi\u0161t\u011bmi a ostatn\u00edmi p\u00edskovi\u0161t\u011bmi. H\u00e1k se za\u010dal zvedat d\u0159\u00edve, ne\u017e se dostal p\u0159\u00edmo nad p\u00edskovi\u0161t\u011b, co\u017e zp\u016fsobilo, \u017ee se p\u00edskovi\u0161t\u011b po zvednut\u00ed ze zem\u011b kym\u00e1cela. Kv\u016fli st\u00edsn\u011bn\u00e9mu pracovn\u00edmu prostoru, \u0161patn\u00e9 viditelnosti a nedostatku bezpe\u010dn\u00e9ho prostoru narazila kym\u00e1cej\u00edc\u00ed se p\u00edskovi\u0161t\u011b do Linovy hlavy. Lin nem\u011bl spr\u00e1vn\u011b nasazenou ochrannou helmu. N\u00e1raz mu zp\u016fsobil zran\u011bn\u00ed a krv\u00e1cen\u00ed do sp\u00e1nku a jeho t\u011blo se reflexivn\u011b shrbilo.<\/p>\n\n\n\n<p>V dob\u011b incidentu zaslechl jin\u00fd d\u011bln\u00edk sl\u00e9v\u00e1rny v okol\u00ed zvuk n\u00e1razu p\u00edskovi\u0161t\u011b do n\u011bjak\u00e9ho p\u0159edm\u011btu a v\u0161iml si n\u011b\u010deho neobvykl\u00e9ho. Okam\u017eit\u011b to nahl\u00e1sil vedouc\u00edmu sl\u00e9v\u00e1rny Lianovi. Oba d\u011bln\u00edci prohledali okol\u00ed a na\u0161li Lina, jak d\u0159ep\u00ed mezi dv\u011bma p\u00edskovi\u0161t\u011bmi, s krv\u00e1cej\u00edc\u00ed hlavou a ochrannou helmou na zemi. Okam\u017eit\u011b informovali hlavn\u00edho vedouc\u00edho spole\u010dnosti Zhenga, kter\u00fd na m\u00edsto sp\u011bchal, aby zorganizoval z\u00e1chrannou operaci. D\u011bln\u00edci dostali pokyn, aby p\u00edskovi\u0161t\u011b od Lina odnesli, a byla p\u0159ivol\u00e1na z\u00e1chrann\u00e1 slu\u017eba (120). Po p\u0159\u00edjezdu z\u00e1chran\u00e1\u0159i s d\u011bln\u00edky na nos\u00edtk\u00e1ch p\u0159enesli Lina do otev\u0159en\u00e9ho prostoru v d\u00edln\u011b k o\u0161et\u0159en\u00ed. Navzdory snaze byl Lin asi po 15 minut\u00e1ch prohl\u00e1\u0161en za mrtv\u00e9ho.<\/p>\n\n\n\n<p>Nehoda si vy\u017e\u00e1dala jednu smrt, nedo\u0161lo k \u017e\u00e1dn\u00e9mu po\u0161kozen\u00ed za\u0159\u00edzen\u00ed a p\u0159\u00edm\u00e9 ekonomick\u00e9 ztr\u00e1ty ve v\u00fd\u0161i 1,31 milionu RMB.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"accident-causes\">P\u0159\u00ed\u010diny nehod&nbsp;<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"direct-cause\">P\u0159\u00edm\u00e1 p\u0159\u00ed\u010dina &nbsp;<\/h3>\n\n\n\n<p>P\u0159\u00edmou p\u0159\u00ed\u010dinou nehody bylo, \u017ee Lin nedodr\u017eoval dostate\u010dn\u00fd bezpe\u010dnostn\u00ed odstup od b\u0159emene a nem\u011bl spr\u00e1vn\u011b nasazenou ochrannou p\u0159ilbu. B\u011bhem provozu mostov\u00e9ho je\u0159\u00e1bu p\u0159i zved\u00e1n\u00ed p\u00edskoven zp\u016fsobilo nespr\u00e1vn\u00e9 \u0161ikm\u00e9 zved\u00e1n\u00ed, \u017ee se p\u00edskovny naklonily a narazily do Linovy hlavy.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"indirect-causes\">Nep\u0159\u00edm\u00e9 p\u0159\u00ed\u010diny &nbsp;<\/h3>\n\n\n\n<ol start=\"1\">\n<li>Selh\u00e1n\u00ed spole\u010dnosti v oblasti \u0159\u00edzen\u00ed bezpe\u010dnosti: Spole\u010dnost pln\u011b neimplementovala sv\u00e9 bezpe\u010dnostn\u00ed povinnosti a postupy \u0159\u00edzen\u00ed v\u00fdroby. Sl\u00e9v\u00e1rna byla \u0161patn\u011b \u0159\u00edzena, s neorganizovan\u00fdm um\u00edst\u011bn\u00edm materi\u00e1l\u016f, nerovn\u00fdm povrchem podlahy a bez vyhrazen\u00fdch bezpe\u010dnostn\u00edch uli\u010dek. Zam\u011bstnanci byli nedostate\u010dn\u011b \u0161koleni a vzd\u011bl\u00e1v\u00e1ni v oblasti bezpe\u010dnosti a nedostate\u010dn\u011b se \u0159e\u0161ily p\u0159\u00edpadn\u00e9 poru\u0161en\u00ed. Krom\u011b toho byla pou\u017e\u00edv\u00e1na nekontrolovan\u00e1 a nekvalifikovan\u00e1 zdvihac\u00ed za\u0159\u00edzen\u00ed.<\/li>\n<\/ol>\n\n\n\n<ol start=\"2\">\n<li>Odpov\u011bdnost spole\u010dnosti zab\u00fdvaj\u00edc\u00ed se zdvihac\u00edmi za\u0159\u00edzen\u00edmi: Spole\u010dnost zab\u00fdvaj\u00edc\u00ed se je\u0159\u00e1bov\u00fdmi stroji dodala zdvihac\u00ed za\u0159\u00edzen\u00ed, kter\u00e9 nepro\u0161lo \u0159\u00e1dnou kontrolou a certifikac\u00ed pro pou\u017eit\u00ed.<\/li>\n<\/ol>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"main-cause\">Hlavn\u00ed p\u0159\u00ed\u010dina&nbsp;<\/h3>\n\n\n\n<p>Hlavn\u00ed p\u0159\u00ed\u010dinou nehody bylo selh\u00e1n\u00ed pr\u016fmyslov\u00e9 a obchodn\u00ed spole\u010dnosti p\u0159i vym\u00e1h\u00e1n\u00ed odpov\u011bdnost\u00ed za bezpe\u010dnost v\u00fdroby a efektivn\u00edm zav\u00e1d\u011bn\u00ed syst\u00e9m\u016f \u0159\u00edzen\u00ed bezpe\u010dnosti. V \u0159\u00edzen\u00ed bezpe\u010dnosti na pracovi\u0161ti panovala neorganizovanost a bezpe\u010dnostn\u00ed kontroly, vzd\u011bl\u00e1v\u00e1n\u00ed a \u0161kolen\u00ed byly nedostate\u010dn\u00e9. Spole\u010dnost tak\u00e9 nedostate\u010dn\u011b d\u016fsledn\u011b prosazovala bezpe\u010dnostn\u00ed provozn\u00ed postupy a neposkytovala odpov\u00eddaj\u00edc\u00ed varov\u00e1n\u00ed a dohled. Lin postr\u00e1dal pov\u011bdom\u00ed o bezpe\u010dn\u00e9 v\u00fdrob\u011b a poru\u0161oval bezpe\u010dnostn\u00ed provozn\u00ed postupy, v\u010detn\u011b nespr\u00e1vn\u00e9ho pou\u017e\u00edv\u00e1n\u00ed osobn\u00edch ochrann\u00fdch prost\u0159edk\u016f.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"secondary-cause\">Sekund\u00e1rn\u00ed p\u0159\u00ed\u010dina<\/h3>\n\n\n\n<p>Spole\u010dnost vyr\u00e1b\u011bj\u00edc\u00ed je\u0159\u00e1bnick\u00e9 stroje, kter\u00e1 byla odpov\u011bdn\u00e1 za instalaci zdvihac\u00edho za\u0159\u00edzen\u00ed zapojen\u00e9ho do nehody, nep\u0159ijala \u00fa\u010dinn\u00e1 opat\u0159en\u00ed k zabr\u00e1n\u011bn\u00ed pou\u017e\u00edv\u00e1n\u00ed za\u0159\u00edzen\u00ed p\u0159ed jeho absolvov\u00e1n\u00edm kontrolou a certifikac\u00ed. Za\u0159\u00edzen\u00ed bylo uvedeno do provozu, p\u0159esto\u017ee nespl\u0148ovalo po\u017eadavky.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"nature-of-the-accident\">Povaha nehody &nbsp;<\/h2>\n\n\n\n<p>Jedn\u00e1 se o typickou nehodu v oblasti bezpe\u010dnosti v\u00fdroby, kter\u00e1 se t\u00fdk\u00e1 speci\u00e1ln\u00edho za\u0159\u00edzen\u00ed a je zp\u016fsobena nepln\u011bn\u00edm povinnost\u00ed spole\u010dnosti v oblasti bezpe\u010dnosti v\u00fdroby, ne\u00fapln\u00fdmi syst\u00e9my \u0159\u00edzen\u00ed bezpe\u010dnosti, nedostate\u010dn\u00fdm bezpe\u010dnostn\u00edm vzd\u011bl\u00e1v\u00e1n\u00edm a \u0161kolen\u00edm, nez\u00e1konn\u00fdm pou\u017e\u00edv\u00e1n\u00edm zdvihac\u00edch za\u0159\u00edzen\u00ed, kter\u00e1 nebyla \u0159\u00e1dn\u011b zkontrolov\u00e1na a certifikov\u00e1na, a poru\u0161en\u00edm provozn\u00edch postup\u016f.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"responsibility-determination-and-recommended-actions\">Stanoven\u00ed odpov\u011bdnosti a doporu\u010den\u00e1 opat\u0159en\u00ed &nbsp;<\/h2>\n\n\n\n<ol start=\"1\">\n<li>Pr\u016fmyslov\u00e1 a obchodn\u00ed spole\u010dnost: Spole\u010dnost \u0159\u00e1dn\u011b neprosazovala sv\u016fj syst\u00e9m \u0159\u00edzen\u00ed bezpe\u010dnosti v\u00fdroby, nedostate\u010dn\u011b poskytovala zam\u011bstnanc\u016fm bezpe\u010dnostn\u00ed vzd\u011bl\u00e1v\u00e1n\u00ed a \u0161kolen\u00ed a \u00fa\u010dinn\u011b nezastavila ani neopravila poru\u0161ov\u00e1n\u00ed provozn\u00edch postup\u016f. Krom\u011b toho neleg\u00e1ln\u011b pou\u017e\u00edvala zdvihac\u00ed za\u0159\u00edzen\u00ed, kter\u00e1 nebyla zkontrolov\u00e1na a certifikov\u00e1na. Jako strana odpov\u011bdn\u00e1 za tuto nehodu se doporu\u010duje, aby m\u00edstn\u00ed \u00fa\u0159ad pro dohled nad trhem p\u0159\u00edpad \u0159e\u0161il v souladu s p\u0159\u00edslu\u0161n\u00fdmi z\u00e1kony a p\u0159edpisy.<\/li>\n<\/ol>\n\n\n\n<ol start=\"2\">\n<li>Spole\u010dnost vyr\u00e1b\u011bj\u00edc\u00ed je\u0159\u00e1bov\u00e9 stroje: Spole\u010dnost vyr\u00e1b\u011bj\u00edc\u00ed je\u0159\u00e1bov\u00e9 stroje nespolupracovala s inspek\u010dn\u00edm org\u00e1nem p\u0159i implementaci po\u017eadovan\u00fdch inspek\u010dn\u00edch postup\u016f b\u011bhem instalace zdvihac\u00edho za\u0159\u00edzen\u00ed. Neleg\u00e1ln\u011b dodala za\u0159\u00edzen\u00ed, kter\u00e9 nepro\u0161lo dohledem a kontrolou.
